Referring New Patients

Effective IMMEDIATELY, we are ONLY accepting new patient referrals generated from an Mount Nittany Physician Group (MNPG) provider for an established patient. This means that we are NOT accepting new referrals from e.g., outside hospitals/health systems or providers (e.g., Geisinger, Penn Highlands, Penn State Health), the Mount Nittany Medical Center Emergency Department, or the Mount Nittany Health Hospitalists upon discharge.

Referrals WITHIN Ambulatory Mount Nittany Health (Expanse):

** Regarding transition of care, AC Clinic will NOT order any anticoagulation therapy for a new patient UNTIL seen in AC Clinic for 1st appt **

  1. Submit an Ambulatory referral to the AC Clinic via Expanse.
  2. Contact Mount Nittany Medical Center Centralized Scheduling (phone: 814.234.6106) to book 1st appointment.
    1. The referring office books the 1st visit for appropriate transition/coordination of care.
    2. Length of 1st appointment is 60 minutes
    3. Review Outpatient Pharmacy Anticoagulation Clinic Policy & Procedure   This policy serves as the collaborative agreement for pharmacy anticoagulation management.

 

Referrals WITHIN Mount Nittany Health Hematology & Oncology:

** Regarding transition of care, AC Clinic will NOT order any anticoagulation therapy for a new patient UNTIL seen in AC Clinic for 1st appt **

  1. Print & fill out the Mount Nittany Medical Center Anticoagulation Physician Referral Form
    1. Fax to 814.234.6775 (AC Clinic).
  2. Contact Mount Nittany Medical Center Centralized Scheduling (phone: 814.234.6106) to book 1st appointment.
    1. The referring office books the 1st visit for appropriate transition/coordination of care.
    2. Length of 1st appointment is 60 minutes.
    3. Patient will meet with the AC Clinic physician medical director.
  3. In addition, fax the following information to the AC Clinic:
    1. Recent past medical history
    2. INRs & warfarin doses over the last 1-2 months
    3. Current medication list
  4. Review Outpatient Pharmacy Anticoagulation Clinic Policy & Procedure  This policy serves as the collaborative agreement for pharmacy anticoagulation management.
