Community Health Needs Assessment

Mount Nittany Health’s commitment to community health is embodied in its mission statement: Healthier people, stronger community.

As a trusted local healthcare leader, Mount Nittany Health is committed to understanding and addressing the most pressing health and wellness concerns for our community. Every three years, Mount Nittany Health conducts a Community Health Needs Assessment (CHNA) and creates a corresponding implementation plan to address the health priorities identified by the CHNA.

The 2025 CHNA was conducted with representation from and active participation of more than 70 human services provider agencies. The report builds upon previous assessments conducted in 2013, 2016, 2019 and 2022 and continues to guide our community benefit and health improvement efforts. The CHNA process is collaborative, seeking input and information from numerous key community stakeholders and the population at large.

Centre County is consistently ranked as one of the healthiest counties in Pennsylvania and the nation by the University of Wisconsin Population Health Institute’s County Health Rankings. While the county overall boasts strong resident health status, experiences of health and social well-being vary across communities. Notably, historical data suggests a potential 14-year difference in average life expectancy between Centre County communities with the lowest and highest averages — highlighting the importance of addressing disparities in health outcomes.

The 2025 CHNA identified three priority areas:

  • Access to care
  • Behavioral health
  • Chronic healthcare and preventative care

In addition to these priorities, the 2025 CHNA introduces a focus on cross-cutting strategies that support long-term community health, including:

  • Community partnerships and collaboration
  • Addressing social drivers of health (SDoH) and the rising cost of living
  • Supporting marginalized and vulnerable communities
