Mount Nittany Health Nephrology has been awarded the Target BP: Silver Award by the American Heart Association (AHA) and the American Medical Association (AMA), recognizing success in prioritizing blood pressure control among its patients. This accolade highlights Mount Nittany Health Nephrology’s commitment to improving community health through diligent hypertension management, a key factor in preventing cardiovascular disease.

The Target BP program is a national initiative launched by the AHA and AMA to improve blood pressure control, with the Silver Award given to healthcare organizations that achieve specific blood pressure targets. Mount Nittany Health Nephrology has demonstrated exceptional dedication to evidence-based practices and patient-centered care, resulting in a marked increase in hypertension awareness and control rates among its patient population.

Mount Nittany Health welcomes patients dealing with hypertension (high blood pressure) to participate in its Hypertension Clinic. The clinic offers education and support to help patients manage their hypertension and maintain a healthy blood pressure range.

Open to all patients, the Hypertension Clinic accepts participants either by referral or by request. Enrolled patients meet with a CRNP for an initial appointment and are then provided with a blood pressure cuff and access to an online platform to track their blood pressure throughout the month. Patients are welcome to participate in the program for as long as needed. This holistic approach to blood pressure control is central to Mount Nittany Health’s mission of “Healthier people, stronger community.”
