Get access to your Mount Nittany Health medical records whenever you need them

Your privacy matters. That’s why Mount Nittany Health takes great precautions to keep your health information safe and secure. With your authorization, there are several convenient ways to request your medical records.

How do I get access to my medical records?

The fastest and easiest way to access your Mount Nittany Health records — for free — is through the MyMountNittanyHealth patient portal. With your account, you can view, download and print your medical records anytime and at no cost.

Request medical records through MyMountNittanyHealth

  1. If you’re a Mount Nittany Health patient and use the MyMountNittanyHealth portal, simply log in.
  2. Then, click on the “Health Record” button at the top of the page.
  3. You can then click on the “Download Medical Record” button located on the left hand side of the page.

If you don’t use the MyMountNittanyHealth portal, you can request your records one of the following ways:

Request medical records electronically

Download, complete and submit a medical records release form electronically through the Patient Request Portal. Please be sure to fill out all fields on both pages of the request form before signing and dating it.

Mount Nittany Health has partnered with Ciox Health/Datavant, the nation’s largest medical information service provider, to process electronic requests securely.

Important information about fees

Personal-use medical record requests made through mail, fax, email or in person may incur fees based on federal guidelines.

Medical records accessed through MyMountNittanyHealth are always free.

How long does it take to get medical records?

Most requests are completed within 2 to 3 business days. But during busy periods, it may take up to 7 business days.

How to request your birth certificate?

Mount Nittany Health does not issue birth certificates. To request a copy of your birth certificate, contact the Pennsylvania Department of Health:
