Mount Nittany Health's diabetes team offers a full range of clinical, nutritional and educational services to provide the highest level of care to patients. All services are tailored to individual needs and goals.

Our experienced diabetes care team of medical professionals includes physicians, nurse practitioners, dietitians and nurse educators. This team of highly trained diabetes experts ensure that patients are fully equipped to manage their diabetes.



Physician

As the leader of our diabetes team, a Mount Nittany Health physician manages the complex processes related to a diabetes diagnosis, including treatment plans and monitoring.

Nurse practitioner

Coordinating care with the diabetes team and other healthcare providers, nurse practitioners also follow patients through evaluations, treatments, medication changes and referrals for care related to the disease.

Nurse practitioners — who can provide guidance on insulin use, track cholesterol and glucose levels and adjust medications, among other tasks — are available five days a week at Mount Nittany Physician Group – Park Avenue.



Dietitian

A diabetes diagnosis often leads to lifestyle modifications, including diet and activity changes. Dietitians educate people with diabetes on carbohydrate counting and sliding scale dose calculation related to intensive insulin regimens.

Nurse educator

Vital in helping teach new skills associated with a diabetes diagnosis, our nurse educators explain to patients and their caregivers how to properly inject insulin, manage hypoglycemia, transition to insulin pump therapy, if needed, continuously monitor glucose and self-manage their diabetes.
